Home Equity Loan Application Checklist:

1. Your 2 most recent pay stubs.
2. If you are self-employed or currently own rental property, we will need your last two years Federal tax returns.
3. A copy of your Deed, Mortgage, or Title Insurance policy.
4. The following information from the lien holder of your home:
   1. Lending institution's name and address
   2. Mortgage account number
   3. Original amount of loan
   4. Current balance and monthly payment amount
5. If your home is paid off, we will need a copy of the Satisfaction of Mortgage.
6. A copy of your Home Owners Insurance policy.
7. A list of your outstanding liabilities containing the following:
   1. Name of institution
   2. Your current balance and monthly payment amount
8. A list of your current assets with the following:
   1. Name of institution
   2. Your current balance
